U18 INDEMNITY, WAIVER & CONSENT

I, (ID No ), hereby certify that I am the parent/legal guardian of (ID No ).
I understand and fully appreciate the inherent dangers in mountaineering, hiking and that recreational activities in the outdoors are potentially dangerous for my child/ward.
I hereby bind myself, my child/ward, my/our dependents, heirs, trustees, executors, administrators, third parties and/or assigns to the terms and conditions of this document. I accept, at my own risk and on behalf of my child/ward, without limiting the generality of the aforegoing, the risks associated with mountaineering, hiking, rock climbing, scrambling, cliff jumping, jumping into pools/rock pools, weather changes, lightning, extreme temperatures, insect and reptile bites, contact with wildlife, physical exhaustion, traversing rough terrain, exposure to heights, crossing of rivers, and the possible consequences of such risks, including, without limiting the generality of the aforegoing, loss or damage to property, illness, injury, mishap and loss of life.
I acknowledge that in the event that my child/ward is injured during any hiking, walking, social club activities and/or workshops arranged by The Centurion Hiking Club, that I will be liable for any and all emergency medical treatment and/or emergency evacuation and that I am solely liable for any such medical and related expenses.
I voluntarily assume and consent to the risks aforementioned on behalf of my child/ward and I accordingly hereby irrevocably waive, relinquish and absolve the Centurion Hiking Club NPC, its directors, exco, members, agents, consultants and/or the persons acting as event organisers, hiking leaders, and volunteers representing the Centurion Hiking Club from any claim or legal proceedings instituted by myself, my child/ward, or any third party of any nature whatsoever for any damage to or any loss of any property and/or any loss or damage as a result of any illness, injury or loss of life which my child/ward may suffer as a result, whether directly or indirectly of their participation in all hiking, walking and social club activities, irrespective of any act or omission, whether negligent or wilful, on the part of the persons.
