APPLICATION FOR U18 MEMBERSHIP 2026

I, (ID No ), being the parent/legal guardian of , hereby submit this application for under-18 membership in the Centurion Hiking Club for the term commencing on 1 January 2026 and ending on 31 December 2026.

In the event that this application is approved, I agree that my child/ward will abide by the Club's Code of Conduct, Guidelines, and Policies, as published on the Club's website, including any amendments or updates that may be made thereto from time to time. I further accept that it is my responsibility to ensure my child/ward remains informed of such updates and to comply with all revisions as they become effective.

I understand and acknowledge that under-18 members must be accompanied by a parent or legal guardian at all times during club events, hikes, walks, and social activities. I undertake to ensure my child/ward upholds the club's culture, objectives, principles, and standards, and all that they embody.

U18 INDEMNITY, WAIVER & CONSENT

I, (ID No ), hereby certify that I am the parent/legal guardian of (ID No ).

I understand and fully appreciate the inherent dangers in mountaineering, hiking and that recreational activities in the outdoors are potentially dangerous for my child/ward.

I hereby bind myself, my child/ward, my/our dependents, heirs, trustees, executors, administrators, third parties and/or assigns to the terms and conditions of this document. I accept, at my own risk and on behalf of my child/ward, without limiting the generality of the aforegoing, the risks associated with mountaineering, hiking, rock climbing, scrambling, cliff jumping, jumping into pools/rock pools, weather changes, lightning, extreme temperatures, insect and reptile bites, contact with wildlife, physical exhaustion, traversing rough terrain, exposure to heights, crossing of rivers, and the possible consequences of such risks, including, without limiting the generality of the aforegoing, loss or damage to property, illness, injury, mishap and loss of life.

I acknowledge that in the event that my child/ward is injured during any hiking, walking, social club activities and/or workshops arranged by The Centurion Hiking Club, that I will be liable for any and all emergency medical treatment and/or emergency evacuation and that I am solely liable for any such medical and related expenses.

I voluntarily assume and consent to the risks aforementioned on behalf of my child/ward and I accordingly hereby irrevocably waive, relinquish and absolve the Centurion Hiking Club NPC, its directors, exco, members, agents, consultants and/or the persons acting as event organisers, hiking leaders, and volunteers representing the Centurion Hiking Club from any claim or legal proceedings instituted by myself, my child/ward, or any third party of any nature whatsoever for any damage to or any loss of any property and/or any loss or damage as a result of any illness, injury or loss of life which my child/ward may suffer as a result, whether directly or indirectly of their participation in all hiking, walking and social club activities, irrespective of any act or omission, whether negligent or wilful, on the part of the persons.

U18 POPIA & CONSENT

The Protection of Personal Information Act (often called the POPI Act or POPIA) is a piece of legislation which governs the law of data protection and privacy in South Africa.

The Centurion Hiking Club will treat, use and process your child/ward's personal information for the purpose, function, management and governance of the Centurion Hiking Club and its activities. The Centurion Hiking Club takes the protection of personal information seriously and is committed to complying with the Personal Protection of Information Act (POPIA).

By submitting this form, I, (ID No ), being the parent/legal guardian of , give Centurion Hiking Club permission to collect and process my child/ward's private information, which includes but is not limited to, receive, record, organise, update, retrieve, consult, and use their information for the purpose of governance and communication pertaining to the activities of the Centurion Hiking Club. This includes, but is not limited to, phone numbers, addresses (physical and virtual), social media, and communication groups, in accordance with the POPI Act.

The personal information collected will only be processed if it is adequate, relevant and not excessive, given the purpose for which it is processed, and if processing occurs in accordance with the relevant provisions of the POPI Act.

In line with this act, I will not use or share the contact details of other Centurion Hiking Club members and hike / event participants for reasons other than club activities, without their personal and express permission to do so.

I acknowledge and agree that, by my child/ward participating in the activities of the Centurion Hiking Club, other participants and/or members may capture my child/ward's likeness in photos and/or video content during events, social events, walks and hikes, which Centurion Hiking Club may publish on social media platforms, including but not limited to, its WhatsApp groups, Facebook page, Facebook group, Instagram, website etc. I undertake to immediately inform the club leaders, participants and/or members taking photos and/or video content in the event that I do not agree to the use of my child/ward's likeness in photos and/or video content on social media platforms. This consent is effective immediately for the membership period of 01 January 2026 to 31 December 2026 and can be withdrawn in the future or where requested or required.

By signing this form, I agree that I understand my child/ward's right to privacy and the right to have their personal information processed in accordance with the conditions for the lawful processing of personal information. I further acknowledge that I understand that I can contact a representative of the Centurion Hiking Club if for any reason I believe that my child/ward's personal information is not being processed in a correct manner.
